  PCA Papillon Rescue will contact your veterinarian. Please contact your veterinarian and give him-her permission to discuss the medical history of your current and past pets. Most of the dogs that come into rescue require veterinary attention so we need to make sure that your vet is willing to  work with us.

 1. Why are you considering fostering a Papillon? Please indicate how you learned about the breed.
 2. Is someone home throughout the day or would dog need to be alone for a part of the day? If you work outside of the home, please be specific with an example of a typical work day including if someone comes home during lunch and who it is, doggie daycare, dog's environment when alone during work week, etc.
3. How will you exercise the foster Papillon? How often? Explain fully.
4. When it is time to transport the foster Papillon to its new home, are you willing to assist in this and to what extent? Would would be willing to fly with the dog? Drive it partway to a new home?
5. How are your pets maintained when you are on vacation or out of town?
6. Do you agree to provide the names and telephone numbers of three (3) references unrelated to you?
7. Do you agree to allow Papillon Rescue to conduct a home visit?
